Attached is our standard application. You may either complete the application OR sign in the designated area on the bottom of a printed copy of this e-mail.
Please note that it is important to include your VIN number when submitting the application; otherwise, we will not be able to issue an insurance card when your policy is approved. It is also important to include driver information for all operators, including date of birth and license number.
Along with your completed application, we ask that you submit the following through U.S. MAIL.
* Original signed application
* Clear photos of collector vehicle (front/side)
* Use/Youth endorsement (attached to this email)
* NJ PIP form (attached to this email)
* Copy of registration, if available
* Premium payment of $100.75 in the form of check or money order.
* Higher rates apply
What the heck is that? Higher Rates Apply.
I'm guessing the Premium is for the year? Right?
